TransAct® TRIGGER MODES
R-WAVE
The computer analyzes the SLOPE and HEIGHT of a positively or negatively deflected QRS complex. Rejects pacer spikes on the basis of sharp rising edge. Automatically initiates ARRHYTHMIA TIMING when at least 8 out of 16 beats are detected as irregular. In this instance, the word ARRHYTHMIA will appear on the screen and the deflation control is inactive. (Refer to Arrhythmia Timing in the section Timing with Arrhythmias).
| |
| This is the pre-set mode. It is the trigger of choice in most instances. |
|
PEAKS
The computer utilizes HEIGHT. It triggers on the first tall event in a complex, the R wave or the pacer spike. DO NOT USE IN THE PRESENCE OF ATRIAL SPIKES.
If an irregular rhythm is identified, the word ARRHYTHMIA will appear on the screen. The deflation control remains active, however, and the clinician may adjust deflation as desired.
| |
| Used as a back up to R-wave. Generally used for patients with wide QRS complexes. |
|
AV PACE
The computer uses the ventricular spike (V SPIKE) of an AV sequential pacer as the trigger event.
| |
| May be used when the patient is 100% AV paced. |
|
ARTERIAL PRESSURE (AP)
The computer uses the SYSTOLIC UPSTROKE of an arterial pressure waveform as the trigger signal. A 14mmHg minimum pulse pressure is required to initially recognize the trigger. Once pumping begins the subsequent pulse pressure requirement is reduced to 7mmHg. Every 64th beat is unassisted and reassessed. This mode is available as an option for clinical situations where an ECG is unavailable or distorted.
| |
| Not recommended for irregular rhythms or irregular pulse pressures. |
|
INTERNAL
The balloon inflates and deflates at a rate of 80BPM regardless of the patient's cardiac activity. This mode is only to be used in situations where there is NO CARDIAC OUTPUT and NO ECG (cardiac arrest, cardiopulmonary bypass). This trigger can only be initiated when the pump is in STANDBY or OFF. Reducing the assist ratio will reduce the internal rate.
